Types of Reports
  • Neighborhood Improvement Zone (NIZ)
  • City Revitalization and Improvement Zone (CRIZ)
  • Military Installation Remediation (MIRP)
Businesses with multiple locations must submit separate reports for each site, using a unique location number for each report. For a single location, the number is "1" and subsequent reports should use "2", "3", or "4", etc.

To gain access to the report, you will need to provide the following information:
  1. Reporting Year
  2. Type of Report (New/Amended)
  3. Location Number
  4. NIZ, CRIZ and MIRP City
  5. Legal Name
  6. Federal Employer Identification Number (FEIN) or Social Security Number (SSN)

Note: The Department reserves the right to review, request, inspect and/or audit the information found within the report.
